Infringement reporting process for copyright, trademark, or other intellectual property

An infringement report notice must contain the following:

  • Identification of the trademark, copyright, or other intellectual property claimed to have been infringed including the name of the intellectual property, the type of intellectual property, the office where the work is registered, the country of registration, and the registration number (e.g. Company, Trademark, USPTO, USA, Reg 2527709).
  • Identification of the material that is claimed to be infringing that is to be removed or access to which is to be disabled, including the URL of the page where it is located, title of the work (if any), and other information reasonably sufficient to permit us to locate the material on the page.
  • A statement that you are the intellectual property owner or that you are authorized to act on their behalf.
  • Your contact information including full name, email address, mailing address and telephone number.
  • A statement that the complaining party has a good faith belief that use of the material in the matter complained of is not authorized by its owner, its agent, or the law.
  • A statement that the information in the notification is accurate and, under penalty of perjury, that the complaining party is authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
  • A physical or electronic signature of the intellectual property owner or authorized to act on behalf of the owner of a copyright that was allegedly infringed.
